Feasibility Study to inform the need for and scope of collocated and integrated health and social care services and intermediate care facilities across WrexhamContract OverviewThe Integrated Rebalancing Care Fund (IRCF) supports the development of two priority areas:1. The delivery of collocated, integrated health and social care services2. Rebalancing the residential Social Care market.This proposal promotes the strategic vision of IRCF, to offer a proactive, coordinated and co-located approach to service provision, focused on the needs of the local population.We are commissioning a feasibility study to inform the need for and scope of collocated and integrated health and social care services and intermediate care facilities across Wrexham.
