Torfaen Family Support Programme aims to provide early help and support for families in need who are below the threshold for statutory intervention. Aims of Service The service aims to support families to achieve the outcomes that they identify. Using a strengths-based approach, the intention is to avoid family needs escalating and therefore reduce the need for support from higher level, more intensive support services including statutory interventions.

Through undertaking a thorough assessment of family strengths and needs, it is possible to develop a Family Support Plan that uses a strengths based approach to set out what the family wish to change, how they will do so, and the support required. The Programme aims to support long term change through an intervention of typically up to 6 months. The role of the support worker is to assess and plan alongside the family. The support worker is also the point of contact for other professionals and for the family members. It is intended that there is a team of Family Support Worker (FSW) roles who are suitably qualified and experienced. A typical caseload for a FSW is 15 - 18 families. The anticipated support period for each family is up to 6 months. Contract Value The funding available for this service is GBP150,000 - GBP160,000 per annum. The length of contract will be 3 years with an opportunity to extend for up to 24 months in total.

Renewal: There is an option to extend the contract for a further period of up to 24 months in totaL.
