Provision of Long Term Placements (shared lives) for adults with disabilities and older peopleWrexham County Borough Council is looking to commission a Shared Lives Adult Placement Service for older people, (age 50+), and for people 18+ with a learning or physical disability, or with mental health needs, on a spot contracted basis.The Shared Lives Scheme (formerly Adult Placement) shall comprise the provision of accommodation and care and support within the home of Shared Lives Carers in the community. The Scheme provides Service Users with an option of living within a family environment in the community.The delivery of Long Term Placements must be in accordance with the requirements in the Social Services and Well-being (Wales) Act 2014 and the associated Code of Practice.We are seeking tender applications from organisations that are able to meet the required service outcomes as detailed below. Although this tender is for the continuation of 3 long term adult placements, we would be interested to learn of provider current market position, what services you can provide, and how you may potentially grow the service. We are particularly interested in learning about any additional funding opportunities providers can bring to help to develop this service.1. The anticipated contract start date would be 1st February 2024. It is to be noted that there are currently 3 long term placements and the hosts would be expected to transfer over to the successful provider.2. This is an outcomes-focused specification for adults 18+. We welcome providers to bring their expertise to the table and outline how they will deliver on the outcomes set out within the specification. Innovation and creativity is encouraged.3. We take a co-productive approach here in Wrexham, and are open to explore all ideas and options about services providers offer, to help to understand market need, especially around respite provision.The Provider will maximise well-being and promote independence of people within shared lives homes delivering a range of support as outlined in the service specificationThe service will be commissioned for three years with a provision to extend by a further two (2) year in one (1) yearly increments, to enable Wrexham County Borough Council to complete a strategic review of Long term Placement services. The service will provide a personalised support plan and rehabilitation package to support the individual involving training and support in relation to improving daily living skills.The provider(s) will be expected to work in partnership with local Social Services Teams, Audiology and Ophthalmology services, and signpost people on to appropriate communication support and wellbeing services as required, in a timely manner.NOTE: The authority is using eTenderwales to carry out this procurement process.
