Glasgow City Health & Social Care Partnership (HSCP) to commission a dementia post diagnostic support service for people with a new diagnosis of dementia for 1 year post diagnosis.

In 2013 the Scottish Government gave a commitment that everybody who received a new diagnosis of dementia would receive 1 year of post diagnostic support (PDS) as of 2015/16; now extended to 2016/17. This commitment is now a Local Delivery Plan target, and as such is delivered by the HSCP. The publication of Scotland's third National Dementia Strategy (expected early in 2017) is expected to reaffirm the commitment to support for people with a new diagnosis of dementia, but may also describe additional flexibility dependent on individual circumstances (for example, diagnosis at a later stage of the disease). The HSCP therefore wishes to retain a degree of flexibility in commissioning the PDS service to ensure that the people of Glasgow are offered appropriate support in line with any updated commitment. In practice, this will require the successful bidder to work closely with HSCP Managers to agree inclusion/exclusion criteria for the PDS service. It is expected that PDS delivered under this tender will continue to be based on the 5 pillar model. Thus the focus is on support to understand and manage the illness; peer support; community connections; and planning for the future and for future decision-making.
