To provide a community based Mental Health Rehabilitation Service, for the purpose of developing or sustaining person-centred support, supported living and community rehabilitation services for people with complex mental health needs within the Midlothian Community.

Midlothian Council is seeking expressions of interest in the provision of a community based Mental Health Rehabilitation Service, for the purpose of developing or sustaining person-centred support, supported living and community rehabilitation services for people with complex mental health needs within the Midlothian Community. Approach The community Mental Health Rehabilitation Service is part of the Wayfinder Graded Support Model. We wish to procure Grade 4 support and accommodation for people in Midlothian with complex mental health problems to prevent people from hospital admission or support people on discharge from hospital. The key elements of Grade 4 support and accommodation are: Graded Support: Providing an environment which "fits" the service users' needs, including the physical environment and staffing relative to the service users' needs, any risks identified and developing meaningful days. Person Centred Choice: Increasing service users opportunities to make choices and continue to develop coping strategies by extending support networks and developing relationships. Meaningful Days: To facilitate service users to establish community living skills by identifying meaningful routines, interests and roles. Services delivering Wayfinder values are: -Recovery orientated services -Evidence Based services -Aspirational services -People and places support rehabilitation Service Delivery At present Castle Rock manage the building on the basis of individual tenancies. There are 7 rooms with en suite toilet and wash hand basin; shared bath and shower room; plus a flat upstairs for one person who is moving towards independent living. The communal areas are lounge, dining room and kitchen. Outside space includes a pleasant garden area. The service is for up to 8 people. Access to the service is for clients who are identified as benefiting from a community based rehabilitation service and are referred and meet the criteria set by the allocation panel. Grade 4 support and accommodation provide a high level of support in the community. Staff are available on site to support people with daily living skills and engage in activities while managing any risks by enhancing the person's coping skills and building relationships. The element of support required will cover, core, overnight and transitional hours to support a person's journey into or out of grade 4 accommodation and support, working closely with health and social care and other third sector providers. Delivery of Grade 4 support and accommodation focuses on the provision of an enhanced community based Rehabilitation Service for people with complex mental health needs. This is supported by a multi-agency team approach to developing a combined support plan with the individual, with a clear process to support them from referral to transition to appropriate accommodation after approximately 12 months. Practical Arrangements There will be a supplier event on Monday 24 September, 10am to 12pm,at Fairfield House, 8 Lothian Road, Dalkeith, Midlothian, EH22 3AA.
