Notice Description
Glasgow City Council is looking to tender for Carers Support Services within Glasgow City Council boundaries. These services will endeavour to instil a person centred and proactive approach to the delivery of carers' services for all individuals who require this provision.
Lot Information
Carer Support Services Glasgow City
The service will be expected to follow the agreed Glasgow Carers Partnership Carer Pathway such as: -Support, facilitate and encourage carers to complete carer self-assessment as gateway to support services. -Undertake proportionate holistic screening assessments with as per current processes with the carer and with family and specialist professionals as necessary. -Support carers to develop Emergency Plans and promote Carer Emergency Card. -Identify levels of needs and risk and carer outcomes. -Work with the carer to develop an outcome based support plan. -Provide support to meet agreed outcomes. -Formally review carer support plans and make decision to close, monitor and review or re-assess if needs change significantly. -Undertake evaluations with carers to evidence impact of service delivery.

Citywide Carer's Information Service-The Carers Information Service (CIS) will offer information and advice for all carers in Glasgow and neighbouring HSCPs across GGCNHS board area. It will also serve as an integral part of the Glasgow Carers Partnership single point of access. -The CIS will address the requirements The Carer (Scotland) Act 2016 introduces on local authorities to ensure that all carers have access to an Adult Carer Support Plan and for Young Carers, a Young Carer Statement. -The CIS will provide a single point of access for adult and young carers. -For health and social care staff and staff in the wider community housing and voluntary sector, the CIS will provide a one stop shop to get information and advice on carers and assist carers to access support. -The CIS will support enquiries and advice in relation to young carers from teachers and pastoral staff from education and other health and social care services. -The service must be available Monday to Friday 9am - 5pm by way of a telephone line. Out with these times, other means of communication should be promoted e.g. email, website contact etc. It cannot provide counselling, advocacy or ongoing case support. The CIS will be responsible for promoting the self-assessment.
