Enhanced Dementia Care
Notice Description
Angus Health and Social Care Partnership would like to undertake a consultation exercise with providers in relation to a model of dementia care which would be provided within a care setting. The client group is people with dementia and complex needs who are clinically fit for discharge from hospital. The service specification will be tailored to the specific needs of this client group with dementia and will require appropriate and safe staffing levels. Training, support and in-reach on a regular basis will be available from the Psychiatry of Old Age Liaison Team. The enhanced care service will not be expected to provide long term dementia care but rather an environment where re-enablement, rehabilitation and person centred interventions will facilitate onward placement to a suitable care environment.

The provision of an enhanced dementia care service within a care setting. The contract will be a block purchase of 6 to 8 beds, ideally within an unit within a care setting. However, where a 6-8 bedded unit is not available, consideration will be given to alternative options eg 3-4 beds in 2 care settings.
