NHSGGC Specialist Adult Learning Disability Services are looking to commission a service to provide support and advice services to people with profound and multiple learning disabilities, their families and carers, and to ensure that people receive the right care at the right time. Specialist Adult Learning Disability Services are provided across six health and social care partnerships and are integrated with social care and have strong relationships with third sector and independent partners. The service will be open to all adult s and young people transitioning from children's to adult services.
Renewal: Option to extend for 4 x 12 months
