Type of service: The service will comprises of three main components: 1. Collecting vulnerable Adults in the main priority given to older people, over 65 years of age, who do not require to be admitted to hospital, from accident and emergency departments and safely transporting them home, or, vulnerable adults who are ready to be discharged home. 2. Assisting the vulnerable Adult into their home and helping them to settle, ensuring their basic needs (with the exception of personal care and medication) are met. 3. Contacting the individual the next working day to offer any additional short-term practical support (for up to 12 weeks with majority of cases closed within 72 hours). Follow-on Support provided incudes: Shopping, income maximisation, wellbeing welfare calls, referring with OTs/ Social workers and Signposting to third sector organisations.
