NHS GG&C requires to commission a HD Specialist service to provide a person-centred, flexible, family-orientated model of care which provides support throughout the many years which people may live with the condition. Key roles of the HD Specialists include holistic assessment, care planning and coordination, assisting individuals to navigate the health and social care system, emotional support and provision of information and advice about HD to patients and those involved in caring for them. Service delivery also incorporates signposting to other services

Lot 1
The purpose of this contract is to commission a service to provide a person-centred, flexible, family-orientated model of care for Huntington's Disease patients which provides support throughout the many years which people may live with the condition. Key roles of the HD Specialists include holistic assessment, care planning and coordination, assisting individuals to navigate the health and social care system, emotional support and provision of information and advice about HD to patients and those involved in caring for them. Service delivery also incorporates signposting to other services
Options: Contract will be awarded for a one year period commencing April 2024. 1 x 12 month extensions may be awarded to the the supplier.
Procurement Information
The market is specific and limited. At time of previous tendering only one supplier submitted a bid. The incumbent supplier Scottish Huntington's Association is the only supplier in Scotland providing specialist, bespoke support to patients diagnosed with Huntington's Disease
