Condition Management (SPF)
Notice Description
This project seeks to commission a health service which can provide condition management advice, occupational therapy style support and expert mental and physical health support for people facing challenges. The provider must work alongside key partners including Dumfries and Galloway Employability and Skills Service, DWP and other employability services supporting people in Dumfries and Galloway, to identify clients who would most benefit from the provision. The provider must be able to provide a flexible person-centred service which includes face to face, telephone and online support to clients based on their specific needs. All staff working on the contract must be qualified to deliver the advice and support and have all the correct registrations and insurance to deliver the programme.
