The Service provides 1 to 1 support for people at an earlier stage of dementia. Delivery of Dementia PDS is currently subject to a Scottish Government Local Delivery Plan (LDP), standard to monitor post diagnostic support delivery - People newly diagnosed with dementia will be offered a minimum of one year's post-diagnostic support, coordinated by a named link worker. The service delivery and implementation to meet the standard is delegated to Health and Social Care Partnerships.
