West Dunbartonshire Council - Pre-tender Engagement for upcoming - Framework Adult Support Services ( Foundations for Change)
Notice Description
WDHSCP are looking to create a Flexible Framework for adult support Services. The Framework requires that the adult support Services must be flexible, responsive and dependable, and must reflect the needs, outcomes and aspirations of both current and future Service Users, their carer(s) and families. West Dunbartonshire Health and Social Care Partnership ("the HSCP") has been working with its strategic partners who are specialists in the delivery of community support services ("Providers") to adults aged 18 to 65 years old who have a range of support needs due to a learning disability complex needs (including behaviours that challenge) physical disabilities cognitive impairment (including acquired brain injury) sensory impairment mental health challenges (including severe and enduring) To develop an outcomes based, flexible framework which is underpinned by Self Directed Support ("SDS") principles. Our Foundations for Change Framework ("Framework"), represents a wholesale shift in the HSCP's approach to commissioning services for residents of West Dunbartonshire who require support to live their lives and achieve the outcomes that are important to them. Historically the HSCP used a transactional model for contracting and commissioning social care services, however, our focus has changed to a more relational based model which concentrates on outcomes and building long term partnerships.The Framework is one of many changes taking place within the HSCP which is aimed at focusing on people's outcomes and increasing people's independence by adopting the Helen Sanderson model of Just Enough Support and redeveloping our approach to social work assessments by evolving our My Life Assessment to focus on what people can do, as opposed to what they can't (deficit model of care and support). The scope of the Framework will cover all SDS Option 2 and Option 3 packages that fall into the abovementioned categories. For the avoidance of doubt, short breaks/respite and day centre services are not covered by this Framework. Supporting adults aged 18 - 65 years old who have a learning disability, complex needs (including behaviours that challenge), physical disabilities, cognitive impairment (including an acquired brain injury), sensory impairment and/or mental health challenges to live healthy lives in their community, is in line with the HSCP's strategic plan for 2023 to 2026 Improving Lives Together. Providers will be able to select the user group that they are able to support throughout the jurisdiction of West Dunbartonshire, via the Invitation to Tender documentation. Providers will be expected to support individuals with a range of complex needs including promoting positive behaviour. Full details of the individual's needs (including any health and/or personal care needs) and outcomes will be contained within the individuals My Life Assessment ("MLA"). The MLA will be shared with the Provider as part of the referral process.
