Health & Social Care Moray intends to procure a comprehensive Unpaid Carer Support Service covering Adult Carers (18+) and Young Carers (under 18, or 18+ in education). The future service will provide accessible, outcome focused information, advice and practical support to help carers sustain their caring role while maintaining their own wellbeing and life opportunities. Delivery will include locality based outreach, telephone and digital support, drop ins, group work (including peer support), carer training, facilitation of short breaks/Time to Live grants, and support around hospital discharge and key transitions. The service must align with the Carers (Scotland) Act 2016, the Health and Social Care Standards, and local strategy for carers in Moray. Lots: - Lot 1 - Support for Adult Carers - Lot 2 - Support for Young Carers Proactive identification of hidden/seldom heard carers; inclusive access (including rural reach and digital inclusion); clear referral and support pathways; rights based information and advice (Carers Act s34); early intervention and crisis prevention; age appropriate support for young/young adult carers; facilitation of short breaks; collaboration with schools, colleges, employers and health partners; support with hospital discharge (s28); transition support from young to adult services; strong data, safeguarding and GDPR compliance; workforce trained in trauma informed and strengths based practice.
