GP Patient Record Audit for NHS Castle Point and Rochford CCG

Notice Description

Castle Point and Rochford Clinical Commissioning Group (CCG) is looking for expressions of interest from provider organisations who might be interested in undertaking a Patient electronic record audit for selected residents of Castle Point and Rochford CCG. The purpose of the audit is to review clinical records for people with a long-term condition and, if required, facilitate a face-to-face health checks by appointment. The scope of the audit is for 1490 individual record reviews; * Phase one being a desktop review of the records; using a provided electronic audit tool. It is anticipated that each review should average at 5 minutes per review; many likely to be less. * Phase two, being the undertaking of any required face to face health checks, where appropriate. The desk top review will require a practitioner who is competent with the use of 'SystmOne' Patient record system and have the skills or access to the required skills to make decisions regarding information within the record and whether a face to face review is required. The anticipated time frame is a total of 12 weeks from commencement of phase one to completion of phase two; phase one lasting no longer than 2 weeks. The CCG may be interested to establish a robust GP Patient Record Audit mechanism across the patch. Interested organisations are invited to express an interest by sending an email to raj.ragiwala@attain.co.uk by no later than 5.00pm Tuesday 14th March 2017 and copy in matt.gillam@nhs.net (Head of Out of Hospital Care).
