Notice Description

NEPRO2 - Transforming Adult Social Care - Reablement Implement project_3715 contract_6183 To achieve the ambition over the next 12 months for reablement services that promote and maximise independence and wellbeing, reducing the need for people to enter long term care. This ambition includes establishing a primary focus on customer outcomes being met by the provision of time-limited 'Step-up' and 'Step-down' services aimed at recovering independence and resident ability to continue to stay in their community for longer. The ASC ambition also includes establishing early, the provision of step-up support to enable customers to remain at home or in the community, to be at least equitable with step-down provision, contributing to the realisation of ASC priorities in prevention and early intervention and reducing the cost of ongoing care packages whilst improving outcomes for residents. This work should result in setting out the early changes required for an optimised reablement function that aims to avoid or reduce the need for long-term ongoing support for residents, where possible and safe, with defined referral pathways from Social Work Neighbourhood Teams, Careline and Hospital Discharge processes.
