Notice Description

On behalf of the two CCGs in Northern Staffordshire , population circa 514,000 , North Staffordshire and Stoke on Trent CCGs wish to commission a Community Falls Service for the North Staffordshire and Stoke-on-Trent CCG population. The Community Falls Service will be expected to reduce individuals falls risks, improve patients' health outcomes and support people to live independently for longer. To minimise the harm due to falls and the number of associated hospital admissions there is a requirement to focus on prevention both primary and secondary. An effective Ccommunity Falls Sservice will coordinate care and will work with a range of organisations to ensure that patients at risk of falling are identified, preventative interventions are delivered and that for those that do fall, multifactorial care plans are in place. In order to deliver the full requirements of the service specification, potential bidders may wish to consider a Prime Provider model and interested parties will need to consider formal partnership arrangements prior to the submission of a collaborative bid. * The contract duration will be for a period of 3 years with an option to extend for up to a further 12 month period. This Tender is subject to the Light-Touch Regime as set out in Schedule 3 of the Public Contracts Regulations 2015 (Annex A). Additional information: If and when this requirement is offered to tender, this will be done via electronic means using the internet. Midlands and Lancashire CSU is conducting this procurement exercise as a central purchasing body for another NHS body with whom the successful bidder(s) will enter into contracts for the supply of the services. Any other public sector body wishing to access the contract may do so only with permission from the contracting NHS body. The contract conditions will be set out in the Invitation to Tender.
