Notice Description

NHS Sheffield Clinical Commissioning Group (CCG) and Sheffield City Council (SCC) have been working in partnership with Sheffield Teaching Hospitals NHSFT (STH) to: * Help reduce emergency admissions to hospital * Support timely hospital discharge, and; * Wherever possible, discharge people to their own home or normal place of residence. Somewhere Else to Assess is a model that supports individuals who are medically fit to be discharged from hospital, but would benefit from a period of assessment in a care home setting while their long-term care and support needs are identified. This model is a key element of the Health and Social Care vision to: * Develop services that keep people well at home * Reduce unnecessary hospital admissions * Reduce the time people have to stay in hospital when they are well enough to leave and; * Only utilise hospital care when clinically necessary and only for as much time as is needed to resolve the acute need. Assessment beds allow 'future needs conversations' to take place outside of an acute setting, reducing the person's risk of infection and/or decompensation in hospital and ensuring discussions about the future don't take place at a point of crisis when their needs are likely to be higher. The aim is always for people to have an opportunity to return home.
