Somerset CCG - Digitalised CHC End-to-End Patient Solution
Notice Description
CONTINUING HEALTHCARE DIGITALISATION EARLY MARKET ENGAGEMENT Continuing Healthcare (CHC) assess some of the most complex and vulnerable patients in our community. They have needs that have arisen as a result of disability, accident or illness so it is vital that their cases are assessed, reviewed and managed in a timely manner. The digitalisation of the CHC Assessment process will result in a drive to increase productivity, speeds in decision making and, in turn, improve the patient and family experience. NHS Somerset Clinical Commissioning Group are seeking to hear from organisations that would be interested in delivering a 'Digitalised CHC End-to-End Patient Solution', where information seamlessly flows between processes that will eliminate ongoing challenges with duplication and interoperability by connecting the entire CHC lifecycle into one platform. This solution would also be required to support CCG integration by the inclusion of other business functions and other patient cohorts from within NHS Somerset CCG. A new e2e solution will give NHS Somerset CCG an opportunity to develop, improve, advance and refresh the current ways of working as well as providing a new CHC model that provides optimal patient outcomes and experience through an innovative digital approach. If your organisation would be interested in providing a demonstration of your solution, please contact simon.matthew1@nhs.net (and cc'ing lisa.phillips29@nhs.net) providing the name of your organisation and the relevant contact details by 31 March 2022. Organisations that take part will not be given any advantage in any potential future procurement that may be undertaken. Commercial confidentiality will be respected. This notice does not represent a formal commitment by NHS Somerset Clinical Commissioning Group to undertake a procurement procedure.
