The banking element will provide holding, managed or third-party accounts, receiving Council payments, making agreed payments, maintaining financial records and retaining invoices and receipts. Provision of Discharge to Assess Services (Night Services)

Location: TauntonBuyer: Somerset County Council
Stage: Tender Date: 5th August 2026 (4 weeks ago) Value: Not specified Source: Find A Tender Service

Description: Pathway 1 in Somerset is the Discharge to Assess (D2A) service and is the route for all supported discharges back to usual place of residence, excluding fast tracked End of Life. The Discharge to Assess model ensures no assessment for ongoing care happens in an acute hospital setting but happens in a person's usual place of residence instead. This enables speedy discharge and prevents the overprovision of long-term care. The ethos of the service is to provide high quality personalised reablement, to be outcome focused, and to enhance what the Individual can do themselves or with help from their support network.

Summary: Somerset Council procured a night-time reablement and intermediate-care service for its Discharge to Assess (D2A) pathway in Somerset. The service supports people discharged from acute hospitals back to their usual place of residence, excluding fast-tracked end-of-life cases. It provides personalised, outcome-focused support so that assessments for ongoing care take place at home rather than in hospital. The aim is to enable speedy discharge, avoid unnecessary long-term care provision and help individuals do as much as possible independently, with support from their own networks where appropriate. The procurement is classified as rehabilitation services and falls within the services category. The work is therefore suited to providers delivering community-based discharge support, reablement and night-time intermediate care for older people and other adults requiring assistance after hospital discharge.
