Notice Description

NHS Leeds West CCG is looking to commission an organisation to provide a social prescribing service, signposting services between GPs and Voluntary Sector Organisations. The service will be provided across all 37 practices within NHS Leeds West Clinical Commissioning Group (LWCCG) and will include self-referral. The service will have a specific focus on LWCCG areas with the highest health inequalities - increasing engagement from areas of high deprivation adopting a proportionate universalism approach. The service will aim to :- - Empower patients and communities to actively self-manage their health issues through peer support. - Provide signposting and increase awareness, support and access about local services and voluntary groups in a managed way. - Provide links to innovative and creative support for patients, which is rooted within patients communities and local networks, reducing isolation and ensuring people live in healthy and sustainable communities. - To expand treatment options, providing greater choice and control for patients, empowering people with the knowledge, skills and resources to promote their own and their families wellbeing. - Improve mental health, wellbeing and quality of life for patients, addressing the wider determinants of health. - To encourage self-management for those with long term conditions providing right information, advice and support closer to home to allow people to live positive lives. All provisions of this service will be expected to meet and comply with NHS standards for IT Governance, Data Protection, security of general and personal information, data and records held and processed and transmitted (via and over all mediums). The successful provider will be required to sign the short form NHS Standard Contract for the service and accept all terms and conditions.
