Notice Description

Suffolk County Council is changing the way in which it commissions home care services. The new Home Care Model features four elements: * Responsive - urgent same day community response where any requirement for on-going home care and support is not yet determined or where a change of needs is being considered, this could be either a short term period of assessment or and offer when required a reablement intervention for people to maximise their independence and enable them to remain at, or return to, home. This will be a redesign of existing in house ACS and CCG services. * Bespoke - for those customers that require staff to be particularly skilled to deliver personal care and support to customers, e.g. particular health conditions, behaviours of concern, or where there is an element of the care package which would not be available in the Locality model. For example; domiciliary overnight support (sleep in and waking night), live in care. * Locality - care providers working in a geographic area, able to meet the planned personal care and support needs of a range of people, when required at a set price. Covers the majority of home care required by vulnerable adults and CHC patients. Tender applications for this have closed, but will reopen later in the year. * Provider of Last Resort - required to meet the statutory duty of the Care Act and have care available as required if market has no capacity, or when a care business fails to help regulate provider behaviour. This will be an in house ACS service. This notice relates to the Bespoke element of the model. Suffolk County Council are seeking the views of the market to help shape the Bespoke service and to inform the service specification(s).
