Quantities shown are an estimate only based on previous usage therefore take up of the whole is not guaranteed. Orders will be placed on an ad-hoc basis for the fixed contract period. Complete the attached statement of agreement and confidentiality agreement (if applicable) and return with quotation. NHS Terms and Conditions apply.
