St Bartholomew's Court in Rye is a scheme for people aged 60 or over who may have long term care needs but are not requiring a substantial amount of regular nursing care.Sanctuary Supported Living are passing over their care provision (but will remain property owner). The desired outcomes are that sustainable, value for money, Care provision is in place that: Fulfil the responsibilities of the Care Act 2014. Improve the health and wellbeing of our population, the quality and experience of health and care services, and keep this affordable. Work alongside health and social care partners to manage demand and service pressures. Meet the needs of Clients and Carers living in St Bartholomew's Court. The desired outcomes for individuals (Clients and Carers) are that: People are supported to be as independent as possible People have access to timely and responsive care People are supported by skilled staff, delivering holistic and personalised care People are kept safe from avoidable harm People are able to live well People are respected and able to make informed choices about services.
