Request for Information - Harrow Referral Optimisation Service

Notice Description

NHS Harrow Clinical Commissioning Group (CCG) is currently operating a pilot Referral Optimisation Service (managing the process and admin of primary care referrals). The current pilot service is due to expire on or before the 25th January 2019. The CCG is launching a process of consultation around potential commissioning options for future service provision. The CCG has previously identified a number of potential benefits relating to referral optimisation: Improving Commissioning Outcomes: Aligning care pathways for better quality of care and patient experience * Strengthen Community and integrated care pathways aligning to the whole systems integrated care * Ensuring referrals are directed to the most appropriate provider, reducing costs and time delays associated with onward referrals * Improve the management of complex patients; identifying patients with complex needs and co - morbidities. * Use of a standard set of referral protocols in line with evidence based medicine best practice guidelines (NICE compliant) * Improving practice access to referral information to help them improve and embed best clinical practice and knowledge of local services Management Information and Business intelligence: To improve the data capture of activity on all referrals in real time Improve the quality of data that is obtained from various business intelligence collecting platforms such as SUS and individual GP practices Contributing to improving patient's experience and outcomes of care primary and secondary care Providing a common, accessible and central point for support and advise for patients on bookings Support delivery of NHS Constitutional Standard targets To develop a body of expertise and guidance about local services The CCG wishes to invite potentially interested providers of referral optimisation services to respond to the attached questionnaire. * Collate considerations from providers' point of view on market development, risk, skills and capacity, etc. * Assess the types of collaboration that might be possible between providers to deliver this service * Gain feedback on the current service model and any alternative solutions Organisations are asked to review the attached Service Specification and complete the attached questionnaire to be returned to Greg Reide; greg_reide@nhs.net and Katherine Emmerton katherine.emmerton@nhs.net by 5pm on 1st March 2018 Please note - Harrow CCG is issuing this notice as a request for information only.
