Notice Description

The service will provide effective management, co-ordination and signposting of High Intensity Users of the local Emergency Department (ED) at Scunthorpe General Hospital (North Lincolnshire). The service will reduce the activity High Intensity Users have on GP Practices and will establish, utilise and coordinate multi-agency and existing professional services to negotiate a reduction in 999 calls. The service aims to demonstrate a reduced workload on unscheduled care services and the wider health economy resulting from reduced 999 calls which otherwise would have attended ED and result in an admission. The service will safely manage and co-ordinate the chaotic and demanding nature of the patient group through the use of multi-agency support and the volunteer sector.
