AT1079 Supported Living for People with Learning Disabilities
Notice Description
Approved Provider List (APL) for Supported Living for Learning Disabilities across 5 Lots. Lot Description 1 Lot 1 - Community Support * A few hours of support per week to develop/maintain independence. * Focused on connecting and enabling and most aspirational in terms of enablement. * Similar to homecare, delivered as an outreach model to help people live independently in their own homes. Providers encouraged to develop this service from existing hubs/schemes if appropriate; or as an extension of their homecare business. 2 Standard Supported Living with variable day hours * Where background support is not required at all times; night support not usually required. Significant potential for support requirements to flex up and down; requires flexible shared hours that are not a barrier to people moving in. * Service users likely to require ongoing support but also needs to be a focus on connecting and enabling people to maximise independence. 3 Standard Supported Living with variable day hours and night support * Where background support is required most of the time during the day and at night.
