Lincolnshire's Children with Disabilities Team, at any given time, have approx. 250 open cases following an Assessment of Need - only a small percentage are Children in Care. Lincolnshire aims to support and meet the needs of children with disabilities within the family home and in their local community environment via:- 1) Specialist activities not requiring a social worker assessment: a) Early Support Learning Provision for 0-5yrs delivered out of Lincolnshire's Children Centres/Family Hubs b) Targeted Positive Activities for 5-18yrs during weekends and school holidays in the community across Lincolnshire. 2) Specialist provision following a social worker assessment and identified need: a) Domiciliary Care and/or social inclusion for 0-18yrs.

Renewal: This contract will likely be procured for an initial 3 years + 18 month option to extend.
