SLAM is seeking to purchase a Caseload Weighting Management Tool / Application with algorithm for predictive analysis functionality (functional or in development) which would enable frontline staff, care coordinators and care teams, plan and prioritise care and resources at a local level based on needs and risk of the patient. The tool should be able to enable: * Support, through supervision to ensure flow through the Community pathway. * Provide drill down facilities from the dashboard view * Provide Patient Access level data i.e. understanding what the key triggers are and other issues surrounding the patient * Outward referrals capabilities internal and external * RAG patients based on needs and risk Please complete the SLAM Caseload Mgmt tool - Supplier Soft Market Test template v1.1 and submit this via Delta
