Dementia Post Diagnosis Support Service

Description: The London Borough of Bromley and the Southeast London Integrated Commissioning Board (SELICB) will be undertaking the recommissioning of the Dementia Post Diagnosis Support Service (Dementia Hub). It is a comprehensive support service for people who have been diagnosed with Dementia, their Carers and the professionals who support them. The Service aim is to improve health and wellbeing, reduce and/or delay the need for costly or intensive services by delivering support in the community. The Dementia Post Diagnosis Support Service is an established pathway from the point of diagnosis to the point when a person becomes eligible for statutory services. Holiday Short Breaks For Children and Young People

Description: The Council is seeking providers of Group Based School Holiday Short Breaks Services for children and young people with disabilities. Informal expressions of interest in the provision of these services are sought at this stage in order to establish the level of market interest. Short breaks are intended to have positive benefits for both disabled children and young people (0-25 years) and their parents and carers.
