The purpose of the CoPs is to provide a forum where colleagues with experience and responsibility in the relevant subject areas can: - Meet together, compare learning and share good practice - to promote productive discussion and encourage collaborative solutions to overcome common challenges - Work together to create materials to help support and embed integrated models of care, and to share learning more widely with colleagues across Wales.",
