Notice Description

The commissioned provider will establish a social prescribing team working collaborative within the multi-agency Single Point of Access (SPOA). The commissioned social prescribing team will use specialist community knowledge to provide information, referrals and signposting to connect people to community groups, activities and assets that will meet their individual needs. The service will help to build the resilience of people and communities and will respond to the high demands for quality acute health and social care services. This service will be funded by Welsh Government Regional Integration Funding (RIF) and will replace an existing grant funded project that delivers similar service outcomes. There will be a requirement for Transfer of Undertakings Protection of Employment (TUPE) impact to be managed between the existing and commissioned providers. The initial contract will be for eighteen months with an option to extend for an additional two years subject to the continuation of Welsh Government funding Bydd y darparwr a gomisiynir yn sefydlu tim presgripsiynu cymdeithasol gan gydweithio o fewn Un Pwynt Mynediad amlasiantaethol.
