Notice Description

The NHS Health Check programme (HC) is a 5-yearly screening programme for adults aged 40-74 aimed at preventing cardiovascular disease and dementia through the early identification and management of cardiovascular disease risk factors. A Health Check consists of an assessment of an individual's risks of developing cardiovascular disease. This requires collecting a range of health information such as height, weight, smoking history, physical activity, blood pressure, and arranging for relevant blood tests. This is a statutory service with an original intention to achieve a 75% uptake rate for the eligible population. Nationally and locally, uptake of NHS Health Checks has been well below that ambition. In Stoke-on-Trent, the programme has historically been delivered exclusively within general practice as a universal offer. We are looking to expand the delivery of the programme and are therefore seeking Providers who can deliver NHS Health Checks in community settings, working with communities and using a targeted approach to reach areas with lower coverage and/or high health needs. Providers will be expected to: * Host regular community events across the city across the calendar year. * Aim to deliver over 1,000 health checks annually and up to 3,000 once fully mobilised. . * Maintain their own clinical record system and complete an agreed data template for NHS Health Check delivery, sharing relevant information with the Local Authority and the individual's registered general practice for clinical follow-up. Providers must demonstrate: * Strong knowledge and experience of referral pathways. * Strong clinical and information governance capability and capacity to ensure that the programme is delivered in a safe and effective way * Relationships or the capability to develop relationships with communities to deliver health checks in settings that will reach target populations. * Flexibility in delivery approaches to encourage uptake of the health check, for example through utilising and continuously improving varied invitation methods. * Capacity to follow up with individuals requiring lifestyle interventions. Please note: The payment method will be via a hybrid method consisting of a lump sum for set up costs and payment by delivery per health check
