GP Out of Hours Service (North Lincolnshire)
Notice Description
Provision of GP Out of Hours services in North Lincolnshire. The service provides GP OOH services, as applicable during an out of hours period, to patients registered in North Lincolnshire where it would not be reasonable for the patient to wait for the services to be available from their registered GP practice during opening hours. The GP Out of Hours Service will * work with health and care system partners across primary, community, mental health, acute and social care to provide support and advice to individuals with the aim of avoiding A&E attendances and hospital admissions, where clinically appropriate. * Provide a face-to-face service and telephone-based treatment and advice to patients where NHS 111 triage has determined that a more in-depth clinical assessment is required by a GP/other professional, or home visit is required. * Provide acute management of exacerbations of long-term conditions, including supporting people to use any prescribed rescue medication in line with care plans. * Provide bookable appointments directly by both NHS111.
