*THIS ADVERT IS FOR MARKET ENGAGEMENT *

NHS Lincolnshire Integrated Care Board (ICB) are seeking to commission Community Connectors & Mental Health and Wellbeing Hubs.

Community Connectors focus on linking together community based organisations at a neighbourhood level, with the aim to connect people to their local community offer, prevent escalation to statutory services and providing individuals with the opportunity to explore different areas of life that might be impacting on their mental health and wellbeing.

Mental Health and Wellbeing Hubs offer a community response for people with mental health and wellbeing issues or distress, providing a non-clinical, safe space to support individuals to have choice, understanding, take control of and manage their own mental health, promote self-efficacy and building knowledge, skills, confidence and resilience.
