Summary of the contracting process

NHS South West London Integrated Care Board, through its Croydon Place services, commissioned community health and social work services for people with long-term conditions. The scope combines long-term conditions community outreach, health and wellbeing checks, the Expert Patient Programme, long-term conditions self-management courses, and Pentathlon healthy lifestyle programmes. The work is intended to raise awareness of risks and risk factors, encourage healthier lifestyles, support earlier identification and diagnosis, and help patients manage their conditions while remaining active and engaged. It also aims to reduce health inequalities and improve access for hard-to-reach Black, Asian and minority ethnic communities, particularly in deprived and underserved Croydon communities including Thornton Heath and New Addington. Delivery is in Croydon, London.

This procurement has been awarded following a below-threshold open competition. It was a single-lot service contract, with electronic submission allowed. Enquiries closed on 5 November 2025 and tender submissions closed on 14 November 2025 at 17:00. The award was made on 1 April 2026 to Asian Resource Centre of Croydon Limited. The active contract was signed on 1 April 2026 and runs from 1 April 2026 to 31 March 2028, with an authority-controlled option to extend by one further year until 31 March 2029. Its awarded value is £247,172.48 excluding VAT, or £296,606.97 including VAT. The competition was evaluated against criteria and weightings set out in the RFQ guidance and bid response documents.

This requirement suits community health providers able to design and deliver preventative, culturally competent support for people with long-term conditions. Credible competitors would need practical experience of community outreach, health and wellbeing checks, patient education, self-management support and healthy lifestyle programmes, with the ability to engage people from underserved and hard-to-reach communities. The work calls for trusted relationships with local communities, particularly Black, Asian and minority ethnic groups, and an understanding of health inequalities in deprived areas. Suppliers should be able to help participants recognise risks, adopt healthier lifestyles, identify conditions earlier and manage existing conditions confidently. The opportunity was suitable for small and medium-sized enterprises and voluntary, community or social enterprises, indicating a delivery model accessible to locally rooted organisations.

Notice Description

The South West London Integrated Care Board (SWL ICB) (Croydon Place) commissioned Long-Term Conditions (LTC) Community Outreach, Expert Patient Programme (EPP), LTC Self-Management and Pentathlon Healthy Lifestyle Programme Services. The aims of the LTC community outreach and EPP programme are to deliver cost-effective, culturally competent, and preventative community health interventions that address the health inequalities in Black, Asian and Minority Ethnic (BAME) communities which constitute circa 50% of SWL ICB's core20 communities, particularly in deprived and underserved wards such as Thornton Heath and New Addington. In particular the LTC community out-reach health and wellbeing checks, Pentathlon healthy lifestyle courses and Expert Patient Programme (EPP) LTC self-management course are anticipated to: * Support raising grass roots awareness of LTC risk and risk factors and support people to adopt healthier lifestyles to reduce the risks of LTC, support early identification and diagnosis and self-management in key Croydon communities. * Empower patients with LTCs to feel more confident in managing their own condition and keep them active and engaged in their lives. * Tackle health inequalities and increase uptake from hard-to-reach groups.
