Mae'r prosiect hwn yn brosiect tai a chymorth holistaidd a dwys sy'n cael ei arwain gan y dinesydd, i ddarparu llety a chefnogaeth i ddinasyddion ag anghenion iechyd meddwl sydd wedi'u heffeithio gan ddigartrefedd. Bydd gan y dinasyddion anghenion cymorth dwys, ac ar lawer ohonynt angen cymorth i ddatblygu sgiliau byw'n annibynnol sylfaenol. Bydd y gefnogaeth a ddarperir yn hyblyg ac yn canolbwyntio ar yr unigolyn, gan gefnogi pobl i oresgyn rhwystrau i ddigartrefedd drwy fynd i'r afael a materion iechyd meddwl, camddefnyddio sylweddau neu unrhyw broblemau eraill y maent o bosibl yn eu hwynebu, gan helpu i wella eu hiechyd a'u lles. I lawer o bobl mae cylch digartrefedd yn mynd law yn llaw a materion cymhleth fel perthynas yn chwalu, cam-drin domestig, cyffuriau ac alcohol, problemau iechyd meddwl a throseddu. Bydd y prosiect yn darparu llety diogelu ac yn gweithio gyda dinasyddion i oresgyn y problemau hyn drwy ddarparu'r gefnogaeth gywir i ail-fagu hyder, hunan-barch a gwella lles meddyliol. This project is a holistic, citizen led, high intensity supported housing project to provide accommodation and support to citizens affected by homelessness who have mental health needs. Citizens will have high level support needs with many needing support to develop basic independent living skills. The support provided will be flexible and person-centered, supporting people to overcome barriers to homelessness by addressing mental health, substance misuse or any other problems they may face, helping to improve their health and well-being. For many people the cycle of homelessness is interwoven with complex issues such as relationship breakdown, domestic abuse, drugs and alcohol, mental health issues and offending. This project will provide safe, secure accommodation working with citizens to overcome these issues by providing the right support to re-build self-esteem, confidence and improve mental wellbeing.

A meet the buyer event for this contract will taking place via Microsoft Teams at 10.00am on 27/01/26 to give potential providers opportunity to learn more about the expectations of the contract and answer any queries. Separately engagement will consult with citizens who are currently supported by the provision, staff from the current providers and any suitable stakeholders. Consultation will be offered via questionnaire and/or face to face engagement.
