Ceredigion County Council is awarding a contract for Mental Health Floating Support Services (Housing Support Grant Funded). The Service can support anyone with either a diagnosis or concerns they may be experiencing Mental Health conditions, to support them to avoid the risk of homelessness, supporting Ceredigion County Council to fulfil its duty under part 2 of the Housing (Wales) Act 2014.itt_121273, itt_121278, itt_121340, itt_121343 & itt_121345 This notice relates to the award of Lot 4 - Specialist Mental Health Floating Support Services. The remaining lots included within the original procurement have not yet been awarded and will be awarded at a later date following completion of the relevant evaluation and approval processes. Further contract award information for those lots will be published in due course to ensure transparency This amendment to the notice relates to the award of Lot 2 - South Ceredigion Mental Health Floating Support Services. The remaining lot(s) included within the original procurement have not yet been awarded and will be awarded at a later date following completion of the relevant evaluation and approval processes. Further contract award information for those lots will be published in due course to ensure transparency. North Ceredigion Mental Health Floating Support Services

The Service can support anyone with either a diagnosis or concerns they may be experiencing Mental Health conditions, to support them to avoid the risk of homelessness, supporting Ceredigion County Council to fulfil its duty under part 2 of the Housing (Wales) Act 2014. itt_121278 is Lot 1 for North Ceredigion Mental Health Floating Support.

Renewal: Potential extension of up to a further 2 years subject to satisfactory performance and agreement by both parties.

South Ceredigion Mental Health Floating Support Services

The Service can support anyone with either a diagnosis or concerns they may be experiencing Mental Health conditions, to support them to avoid the risk of homelessness, supporting Ceredigion County Council to fulfil its duty under part 2 of the Housing (Wales) Act 2014. itt_121340 is Lot 2 for South Ceredigion Mental Health Floating Support.

Renewal: Potential extension of up to a further 2 years subject to satisfactory performance and agreement by both parties.

North & South (Lot 1 & Lot 2) Ceredigion Mental Health Floating Support Services

The Service can support anyone with either a diagnosis or concerns they may be experiencing Mental Health conditions, to support them to avoid the risk of homelessness, supporting Ceredigion County Council to fulfil its duty under part 2 of the Housing (Wales) Act 2014. itt_121343 is Lot 3 for both North & South Ceredigion Mental Health Floating Support. - Providers may ONLY submit for this Lot if they have also submitted for Lots 1 AND 2. - Lot 3 will only be reviewed if a single supplier is successful for Lot 1 AND Lot 2. Lot 3 is not scored and enables providers to outline any discounts or cost reductions that they can offer should they be successful in submissions for both Lots 1 AND 2.

Specialist Mental Health Floating Support Services (All Ceredigion)

The Service can support anyone with either a diagnosis or concerns they may be experiencing Mental Health conditions, to support them to avoid the risk of homelessness, supporting Ceredigion County Council to fulfil its duty under part 2 of the Housing (Wales) Act 2014. itt_121345 is Lot 4 for Specialist Mental Health Floating Support for the entirety of Ceredigion.
