An Arts & Health programme (visual arts and crafts/mixed media programme) will be offered to patients who regularly present at GP surgeries in Ceredigion, Pembrokeshire and Carmarthenshire living with a chronic condition and/or pain, social isolation and mental health difficulties as a result of circumstances/condition to improve health and wellbeing. The project will be delivered by an experienced arts organisation/s with a track record of delivering high quality arts provision to improve health and wellbeing.

The project will be delivered by an experienced arts organisation/s with a track record of delivering high quality arts provision to improve health and wellbeing.
