Notice Description

The Home, Settle and Support service provides short-term, practical support to vulnerable residents following a hospital stay, Emergency Department presentation, or where they are at risk of hospital admission from the community. The service supports timely and safe discharge, helps people regain confidence and independence at home, reduce loneliness and social isolation as well as prevent avoidable readmissions. Support is tailored to individual need and normally provided for a short period, up to a maximum of four weeks, with clear signposting and warm handovers into relevant community and voluntary sector services. We are inviting the market to help us develop our future iteration of the service through shared experience, best practice and innovation.

We are seeking insight from providers to inform service design, delivery models, innovation, and best practice. Your responses will help shape the future specification and procurement approach. Completed questionnaires should be returned no later than 4pm, 3rd July 2026. Market Engagement date We will be holding a virtual market engagement session to suppor the process on the 7th of July. The session will seek to provide an overview of the the current service, initial proposals for the future service models, outline of the procurement timelines and next steps and provide an opportunity to ask questions and seek clarification.
