Description: The Home, Settle and Support service provides short-term, practical support to vulnerable residents following a hospital stay, Emergency Department presentation, or where they are at risk of hospital admission from the community. The service supports timely and safe discharge, helps people regain confidence and independence at home, reduce loneliness and social isolation as well as prevent avoidable readmissions. Support is tailored to individual need and normally provided for a short period, up to a maximum of four weeks, with clear signposting and warm handovers into relevant community and voluntary sector services. We are inviting the market to help us develop our future iteration of the service through shared experience, best practice and innovation.
