Fairfax Multi Academy Trust is seeking quotations from suitably qualified providers for the provision of Occupational Health and Pre-Employment Medical Services. The Trust wishes to appoint a single provider capable of delivering both services through a single contractual arrangement and, where possible, a single integrated platform. The Services include, but are not limited to: * Occupational Health management referrals. * Long-term sickness absence assessments. * Frequent absence reviews. * Return-to-work assessments. * Fitness-for-work assessments. * Stress and wellbeing assessments. * Disability and Equality Act related assessments. * Workplace adjustment assessments and recommendations. * Pre-employment health questionnaires. * Pre-employment medical screening and assessments. * Occupational Health clearance recommendations. * Reporting and management information. * Account management and support. The Provider shall deliver a secure online solution that supports referral management, case tracking, reporting, management information and pre-employment medical screening. Estimated annual volumes include approximately 50 Occupational Health referrals and 60 pre-employment health questionnaires per annum. Suppliers must be capable of providing both Occupational Health Services and Pre-Employment Medical Screening Services under a single contractual arrangement. Following evaluation of written submissions, the three highest-scoring suppliers may be invited to participate in a supplier demonstration.

Renewal: The Contract will be awarded for an initial term of 12 months. The Trust reserves the right, at its sole discretion, to extend the Contract by up to two further periods of 12 months, subject to satisfactory performance, continued business need and funding availability. Any extension will be confirmed in writing by the Trust prior to the expiry of the Contract Term.
