In Greater Manchester there are an estimated 120,000 people experiencing Multiple Disadvantage. This is defined as a combination of homelessness, substance misuse, mental health issues, domestic abuse and contact with the criminal justice system. People facing multiple disadvantage are at the sharp end of socio-economic and health inequalities. As part of Greater Manchester's Live Well commitment to tackling health inequality, we want to build on the existing Inclusion health service to reduce this inequality. GMCA are looking for providers to deliver two distinct services which will remove a range of different barriers for people experiencing MD to having their health needs met. Lot 1: Primary Care Led Support- Deliver support in a primary care setting to people facing Multiple Disadvantage, in Rochdale, Oldham and Manchester.
