Notice Description
The Home from Hospital service will support safe and timely hospital discharges, by accompanying patients back home on discharge and supporting them with shopping, topping up utilities and befriending, to reduce the risk of readmission.
Planning Information
Liverpool City Council is seeking to engage with the market in advance of procuring a Home from Hospital Service to support Liverpool residents following discharge from hospital. The service will aim to facilitate safe and timely hospital discharge, reduce the risk of readmission, promote independence, and support individuals to regain confidence and reconnect with their communities. The Council is inviting providers, voluntary and community sector organisations, social enterprises, and other interested stakeholders to participate in pre-market engagement activities. This engagement will help inform the future service model, delivery requirements, outcomes, and procurement approach. Suppliers will have the opportunity to provide feedback on service design, innovation, partnership opportunities, and market capacity. The engagement is for information-gathering purposes only and does not form part of any formal procurement process. Any future procurement opportunity will be advertised separately in accordance with the Council's procurement requirements.
