The notice relates to the provision of Enhanced Primary Care Homeless Service in Lewisham This notice is an intention to award a contract to the existing provider following direct award process C. The lifetime value of this contract is PS500,000 (PS125,000 per annum) The contract term is 2 years with the option to extend for a further 2 year at the sole discretion of the Authority. The contract will start on 01/04/2027 and end 31/03/2029, if the optional extension is utilised the contract end date will be 31/03/31

Lot 1

This specification should be considered as part of the wider homeless health pathway. The service provider is expected to work in partnership with homeless agencies (as locally commissioned) as well as a range of health, social care and voluntary services to ensure a seamless and equitable service for homeless patients. The service provider shall: * Provide clinically led outreach clinics across multiple sites located in a community setting providing primary care health services to the homeless population. * Designated sites have been identified through collaborative engagement across SEL ICB, LBL colleagues (Housing teams, rough sleeping teams, health inclusion teams) and housing providers. The provision of outreach clinics may be considered through sub-contracting arrangements and as per any agreements with SEL ICB. * Ensure that there is a named GP identified as clinical lead to provide clinical oversight of the service. * Ensure that all clinics are clinician led and delivered by a GP or an Advanced Nurse Practitioner (including prescribing capabilities) * More than 1 clinician can be identified if covering different sites. Clinicians should have experience and/or an interest in working with the homeless population. * The service model should offer continuity of care through designated clinicians across sites. * Identify any additional workforce as they see fit for delivery of the homeless care model * Have the capabilities to flex the service model and respond to service delivery demands to meet the service users needs and in agreement with SEL ICB. * Lead on and attend frequent Multi-Disciplinary Team (MDT) meetings with multiple agencies actively involved in the health and wellbeing of the resident(s). Frequency to be determined by the provider and key stakeholders. * Delivery of annual webinar session and / or information and guidance for homeless service provider staff in supporting their homeless clients in accessing health services. * Participate in development of new models of care, shared protocols and pathways for homeless health in line with local strategies and commissioning intentions. * Develop good relationships with relevant services and agencies to ensure effective delivery of care. * Ensure that there are robust business continuity arrangements in place for incidences of annual leave or sickness. * Ensure that they are effectively able to implement flexible and alternative models of care in response to any local or national incidences e.g. pandemic ensuring that there is continued access and flexibility in service provision and that this is clearly communicated to all service users and key stakeholders. * Ensure delivery of the service aligns to the governments core principles for health and care professionals25
