Notice Description

The purpose of this supported living service is to enable adults who are homeless with complex needs including substance misuse issues to achieve their full potential. This includes improved mental and physical health; emotional, psychological and social needs being met and reducing the harms associated with substance misuse. The Provider will achieve this by the sensitive recognition of the potential in each Individual and by understanding that it may change over time. The service will provide both long-term and medium-term accommodation where Individuals will have appropriate legal tenancies, domiciliary care and support. The Provider will be required to work in partnership with the individual, the City Centre Social Work Team to identify and refer on to appropriate lower support accommodation including the general Housing waiting list, extra care provision or private rented accommodation. The care and support practices involve caring about and for an Individual's physical, intellectual, emotional, psychological and social requirements, and should do so in ways that maintain dignity, are as unobtrusive as possible, and seek to ensure freedom from discomfort, boredom, insecurity, anxiety and isolation.
