Award of Quick Step Service
Notice Description
Clackmannanshire Council would like to request a quote from their existing Care at Home framework providers to deliver a 'Quick Step' service from Monday 7 August 2017 to Saturday 31 March 2018 with an option for Clackmannanshire Council to extend this contract by a further year (to 31 March 2019) and then potentially by an additional one year (to 31 March 2020).Contract will be awarded to one provider for this period.The service is to be available to Clackmannanshire Council for 15 hours a days, 7 days a week.The times of operation each day will be:-7 am to 10 pm 15 hoursThe service is to cover the full geographical area of Clacksmannanshire Council.This service will facilitate 'stepping down' from hospital services and help avoid people 'stepping up' as hospital admissions. It is envisaged that a substantial element of the service is likely to be for people who have been assessed as ready to be discharged from hospital, or are otherwise entering the care system following a crisis.The Quick Step service will provide personal care to service users by giving a high level of care providing short visits to service users' homes. This service will be an additional service to the existing care at home framework service and, in the main, to support planned discharges from hospital.The care packages will be reviewed on a regular basis with an intention of reducing input as independence of the service user increases.The service will be characterised by the quick, responsive interventions required within 24 hours of referral.The Quick Step service will provide timely personal care and keep Social Services fully informed on the care needs of the individuals in receipt of this service.A joint assessment between Quick Step and Social Services will be carried out within 5 days of the service commencing. This will allow for a quick agreement to be reached with all parties, including the service user and their family.Each individual will likely receive a maximum of 2/3 weeks of visits which are intended to promote confidence on the part of service users by providing a quality pathway out of hospital. The service to the individual may, in some instances, be extended after agreement with Social Services
