Practitioners will work as part of the primary care team and will work in close partnership with a wide range of community based services within Midlothian.

    "description": "Midlothian Council is seeking expressions of interest in the provision of a Wellbeing Service. Approach The Wellbeing Service is part of an approach in Midlothian that promotes joined-up, collaborative working that supports self-management. It is in line with the House of Care Model which: -Focuses on conversations about what is important to people to help them recover or live well with their health conditions. -Puts people at the centre of their care. They express their own needs and decide on their priorities. -Supports self-management and collaborative relationships between patients and professionals. People should have the knowledge, skills and confidence to manage their condition effectively in their everyday life. -Tackles health inequalities. -Involves each individual in a unified, holistic care planning process. A common set of relevant skills and processes reduces the burden of training. Service Delivery The Midlothian Wellbeing Service will provide 1:1 support to people who are facing challenging issues that affect their health and wellbeing. People will be supported to focus on their personal outcomes and manage their own health and wellbeing, by providing time and space for 'good conversations' about what is important to them and how they can move forward in their life. These conversations will harness the role of the person: their strengths, networks and community supports. Some people will also be supported to participate in a structured group programmes led by Wellbeing Practitioners. In addition, it is anticipated that peer volunteers will be part of the Service. The Wellbeing Service is expected to work in close partnership with a wide range of community based services to ensure that people are able to access the most appropriate support available within Midlothian. Most people who will be referred will have complex social issues, mental health difficulties and/or a long term condition. It is expected that over 1,200 people will be referred each year and that 70% or more will engage. Some people will require just one or two appointments, others around six. Practical Arrangements The Wellbeing Practitioners will work collaboratively with the local GP Practice Team and will be part of a broader 'House of Care' network of organisations in Midlothian. The service will be operational in all 12 GP Practices in Midlothian from 1st October 2018. (The service has been operational in 8 GP practices for over a year.) A Practitioner team with a skill mix would be welcomed. However it is expected that a number of Practitioners will be qualified in a health or social care related discipline. There will be a supplier event on Monday 14th May, 10.00am to 12.00pm at Fairfield House, 8 Lothian Road, Dalkeith, Midlothian, EH22 3AA.
