The service must be: informed by current evidence, incorporating the three elements which are essential to weight loss - dietary advice, behaviour change and physical activity; compliant with statutory requirements; free at the point of contact; and target those with the greatest health needs.

Lot 1

The Community Weight Management Service will principally include patients with a BMI>25>40 (up to a maximum weight of 240kg) and initially target patients with existing chronic disease and weight related health concerns e.g. specific co-morbidities or impaired mobility. Patients will be 'recruited and referred' via Primary Care / Community Health teams; Acute clinical teams and Self Referral for established CHD, Stroke and Diabetes). The community service excludes patients where more intensive clinical support is more appropriate such as patients with severe obesity; who have complex co morbidities or have significant additional social needs who will receive NHS Specialist Weight Management Care. All referrals are received through a NHSGGC managed weight management admin hub where demographic information, medical history, additional needs are considered before identifying a weight management service destination. The admin hub transfers necessary contact information to the community weight management provider twice weekly including preferred group location.
