Trusted Assessor' schemes are a national initiative designed to reduce delays when people are ready for discharge from hospital. West Northamptonshire Council (WNC), in partnership with the Integrated Care Board (ICB) and Northampton General Hospital (NGH), is jointly commissioning a Trusted Assessor model to improve discharge pathways, reduce delays in transfers of care, and strengthen multi-agency working across adult health and social care services, with WNC acting as Lead Commissioner for the contract. Following a period of in-patient care, and prior to an individual's discharge, a statutory assessment, or review is required, to establish eligibility and level of need. Currently, care providers assess the individual in the hospital setting, using their own assessment tools however, due to capacity and resource issues, this can contribute to delayed discharges. The purpose of the Trusted Assessor is to support and facilitate discharges into care providers and to avoid discharge delays from NGH. The model will play a crucial role in ensuring timely, consistent, and person-centred assessments across acute and community healthcare settings, fostering improved outcomes for individuals and service systems alike. BACKGROUND TO NPH Northamptonshire Partnership Homes (NPH) are an arm’s length management organisation, formed in January 2015 that manages over 11,500 homes on behalf of West Northamptonshire Council (WNC). NPH is a wholly subsidiary of WNC (previously Northampton Borough Council prior to the change to establishment of WNC as a unitary authority in April 2021). NPH are a Company Limited by Guarantee (CLG) and a not-for-profit organisation. This means that our income is put back into providing housing and housing related services and supporting the communities we work in. OPPORTUNITY OVERVIEW NPH are looking for an independent Lift Consultant that can undertake necessary works for a replacement/upgrade programme on lifts within its managed properties over a 5 - 10 year programme. The consultant would be required to carry out site surveys, produce technical specifications/designs for the works, prepare pre-construction information in preparation for a tender exercise to be completed by NPH. Once the contract is awarded the consultant will be required to overview the project until completion with meetings held with the appointed contractor before, during and after installation of any lifts including witnessing the final test of newly replaced or upgraded lifts.
